Case Management Services for Older Adults now offered from Jewish Family Services - Raleigh

ImageJewish Family Services provides professional, affordable and compassionate case management services to older adults. Our social workers will work with you to feel empowered, to maximize the quality of your life, and reach your full potential with dignity, hope, and strength. 

Concerned about the challenges of aging but not sure where to start or what to ask?

Trying to decide what changes or help is needed to maximize your quality of life?

Need to make decisions but having difficulty navigating options are available and affordable?

Worried about your parents' ability to manage in their home or current living situation?

Need information, guidance and support in how best to care and help your aging parent?

JFS-Raleigh provides social work services to older adults and their families with a focus on quality aging to maintain their highest level of independence and dignity while remaining safe. Our social worker provides social services on an affordable sliding scale to meet the needs of older adults in our community. These services include:

  • Phone or virtual visits via Zoom to assess needs and develop a plan to meet identified needs.
  • Exploring options for older adults and their families.
  • Consultations with older adults, as well as adult children to provide support, education, guidance, and available resources.
  • Linking older adults to resources.

What’s new?  We are now The Sunshine Team!                                                           

                                                                                                                             

Thanks to a generous grant from Carolina Foundation for Jewish Seniors, we are able to expand our program!

We will continue to provide meals for congregants after discharge from the hospital or rehab but now are able to provide meals for up to 1 week. The food will be specific to one's dietary needs, with input from a nutritionist as necessary.

A new and major focus of The Sunshine Team will be reaching out to those seniors who may be isolated, live alone, or have a chronic illness that limits their ability to get around as much as they’d like. Some may need new opportunities to socialize safely or need other assistance.

Our first goal is ensuring that our seniors are able to connect online. We want everyone to have the opportunity to attend online events, keep in touch with family or friends, or attend services. All of these can be done via Zoom. Online programs geared to our senior population are also in the works. We can also help with needs such as errands or grocery shopping for those who have difficulty doing so themselves. There is no charge for these services.

Participation is voluntary and the privacy of our congregants is of the utmost importance. Requests can be made by calling the synagogue office, Rabbi Bender or Michelle Bannon at 706-832-3048.                                                                                                                                       

We also need volunteers to help in all aspects of this program.

Donations to help cover costs are appreciated and may be made on ShulCloud or by check to Bnai Israel with "Sunshine Meals" noted on the memo line.
